Who this letter is for

Request an itemised bill and challenge duplicate, incorrect or unexpected medical charges. It is written in the formal register companies, landlords and card issuers expect, and it records the facts, the remedy sought and the deadline in a way that supports any later escalation to an ombudsman or the courts.

How to use this template

  1. 1Request an itemised bill.
  2. 2Compare it with the Explanation of Benefits.
  3. 3Mark each disputed line.
  4. 4Attach only necessary supporting records.
  5. 5Request an administrative hold during review.

What to include before you send it

Re: Formal dispute of medical bill dated [DATE] — account [NUMBER]

Dear Billing Department,

I dispute [AMOUNT] of the bill for services on [DATE]. The charge is disputed because [it is duplicated / the service was not received / the code or quantity appears wrong / it conflicts with my insurer's Explanation of Benefits / it may be an eligible surprise out-of-network bill].

Please provide a complete itemised statement showing all billing and procedure codes, payments, contractual adjustments and insurance submissions. Please also explain [SPECIFIC LINE ITEMS].

While this dispute is reviewed, place the account on administrative hold and do not send the disputed amount to collections or report it to a credit bureau. I request a corrected bill and written decision within 30 days.

If the charge involves out-of-network emergency or facility care, please also review it under the No Surprises Act and applicable state protections.

Sincerely,

[Your name]
